Sleep Medicine Team

Our sleep medicine program is supported by Registered Polysomnographic Technologists (RPSGTs) — nationally credentialed experts trained in the science of sleep. They specialize in:

  • Sleep study interpretation

  • CPAP education and troubleshooting

  • Identifying patterns that affect breathing and rest

  • Supporting patients through diagnosis, treatment, and long‑term adherence

Their work helps patients breathe easier, sleep better, and regain energy — often transforming quality of life in ways that families immediately notice.

Advanced Illness & Support

Compassionate Care From a Team Built for Moments That Matter

Our Advanced Illness team supports patients and families through some of the most challenging chapters of their healthcare journey. Their work is grounded in dignity, clarity, and the belief that no one should navigate serious illness alone.

This team brings together a palliative physician, registered nurses, a home health aide, and a social worker — each offering a different kind of strength, all working in harmony.

What This Team Provides

  • Comfort‑focused medical care led by a physician trained in palliative and supportive medicine

  • Nursing support that brings calm, clinical skill, and steady guidance

  • Hands‑on assistance from a home health aide who helps patients feel safe and cared for at home

  • Emotional and practical support from a social worker who helps families navigate decisions, resources, and changing needs

A Team Built Around Humanity

Together, they manage symptoms, support families, guide difficult conversations, and create a sense of stability during uncertain times. Their presence is gentle but strong — the kind of care that helps patients feel seen, heard, and respected.

This is the heart of Reliant’s Advanced Illness program: a team that blends expertise with compassion, and clinical skill with genuine human connection.
